Hormonal Sensitivity Across Reproductive Life Stages: Why Bother?

Hormonal Sensitivity Across Reproductive Life Stages: Why Bother?

This is an IAPMD Professional Community Webinar, and the content is aimed at Health Care Professionals & Researchers. This webinar will specifically benefit all health professionals, people with lived experience, educators, trainees, etc.

Pο»Ώlease join IAPMD and Dr. Benicio Frey as he discusses hormonal sensitivity across reproductive life stages.

Multiple lines of evidence show that a subset of people are more vulnerable to experiencing intense mood, anxiety, and other psychological symptoms during times of (physiological) hormonal fluctuations, such as menarche, premenstrual period, ovulation and perimenopause.

In addition, converging evidence from multiple studies shows that these episodes can lead to severe emotional distress, disability, and a higher risk of self-harm. Despite that, the concept of hormonal sensitivity is not well disseminated in the educational curricula, which contributes to poor health care outcomes.

After attending this webinar, participants will be able to:

  • Review and discuss the biological studies on hormonal sensitivity and reproductive mental health.

  • Review and discuss the clinical studies on hormonal sensitivity and reproductive mental health.

  • Review and discuss the literature of premenstrual worsening as a comorbid condition versus an exacerbation of other disorders.

  • Review the link between the menstrual cycle and suicide risk and behavior.

  • Discuss the educational and health service gaps in this area.

WE ARE INTERNATIONAL - This webinar will start at 1:30PM EST Find your timezone here.

Sο»Ώpeaker: Benicio N. Frey, MD, PhD, FRCPC (he/him)

Dr. Frey is a Professor in the Department of Psychiatry and Behavioural Neurosciences at McMaster University; Medical Director of the Women’s Health Concerns Clinic at St. Joseph’s Healthcare Hamilton; Academic Head of the Mood Disorders Program, McMaster University; and Director of the Centre for Clinical Neurosciences at St Joes/McMaster University.

His research is primarily focused on understanding the neurobiology of mood disorders, with a focus on female-reproductive life events. Some of his current research involves clinical trials in the areas of premenstrual dysphoric disorder, perinatal anxiety, and perinatal emotional dysregulation, as well as studies of sleep and biological/circadian rhythms disruption across pregnancy and the postpartum period. He has more than 300 publications accepted in peer-reviewed journals and has received numerous career awards, including the title of University Scholar in 2020.

Despite the documented success of GnRH agonists for the treatment of treatment-resistant premenstrual dysphoric disorder (PMDD), many patients struggle to find providers who have sufficient knowledge of PMDD and its evidence-based treatments and/or who are comfortable treating PMDD after first-line treatment options have failed.

This session will highlight the barriers to initiating GnRH agonists for treatment-resistant premenstrual dysphoric disorder (PMDD) and offer practical solutions to address these barriers for providers who encounter patients with treatment-resistant PMDD but may not have the necessary expertise or comfort with providing evidence-based treatments.

This session will also include:

  • example patient and provider handouts,

  • screening tools,

  • treatment algorithms to help provide guidance for implementing GnRH agonists into practice amongst a broader scope of clinicians, including general psychiatrists.

Sο»Ώpeaker: Melissa Wagner, M.D., Ph.D.

Following residency, Dr. Wagner took an academic attending psychiatrist position as a reproductive psychiatrist and, through peer supervision and clinical and research collaborations, gained expertise in treating patients with mental illness during times of hormonal transitions. These times of hormone transition include prior to, during, and after pregnancy, mental health disorders related to hormone changes or the menstrual cycle, including perimenstrual dysphoric disorder (PMDD) and menopause, and mental health disorders related to or affecting fertility. Her triple board training serves as a significant asset in bridging the clinical gaps between reproductive psychiatry and family mental health. Dr. Wagner collaborates with internists, obstetricians, reproductive psychiatrists, and psychologists at both the University of Cincinnati and the University of Illinois with an academic focus that includes developing innovative care delivery models to help improve the diagnosis and treatment of reproductive-related mental illnesses and research involving PMDD, suicide risk, and neurosteroids. Finally, Dr. Wagner is clinically active in both psychiatry and pediatrics and serves as a supervising pediatrician within a Medicine-Pediatrics resident clinic at the University of Cincinnati and as the collaborative care reproductive and child psychiatrist in an innovative two-generation clinic within the internal medicine/pediatrics department at the University of Illinois in Chicago.

Alcohol Use and Motives for Drinking Across the Menstrual Cycle

Patients with a special interest are welcome to attend but please note, that the presentation will be clinical in nature and we are not able to answer questions about individual health concerns/circumstances. Patient tickets are available here >>

Females and AFAB individuals who drink alcohol experience high rates of negative physical and mental health consequences, and high-risk drinking in females is rising in the United States. Existing findings are inconsistent but suggest a relationship between ovarian hormones and alcohol use. This session will describe how alcohol use and drinking motives vary across the menstrual cycle in a large, transdiagnostic sample of psychiatric outpatient females.

As the largest prospective study of alcohol use and the menstrual cycle in a clinical sample to date, our findings encourage individual assessment of cyclical changes in alcohol use in females who may be at risk of negative health outcomes.

Speaker: Jordan Barone, B.A.

Jordan is a fifth-year MD/Ph.D. student at the University of Illinois at Chicago, where she is pursuing both her MD and a Ph.D. in Neuroscience. Jordan is passionate about reproductive mental health and neuroscience, and her Ph.D. work seeks to understand the relationship between ovarian hormone flux and cyclical exacerbation of suicidal symptoms by identifying biomarkers of symptom change across the menstrual cycle. She graduated from Vanderbilt University with double majors in Neuroscience and Child Development, then completed a post-baccalaureate research fellowship at the National Institute of Mental Health (NIMH), studying sex differences in brain development across puberty. Her research interests focus on biomarkers at the intersection of hormones, brain, and behavior, such as neuroactive steroids and changes in the GABA-A receptor.

Understanding When and Why Some Individuals Attempt Suicide: Testing an Emerging Framework Integrating Menstrual Cycle Fluctuations in Risk

Understanding When and Why Some Individuals Attempt Suicide: Testing an Emerging Framework Integrating Menstrual Cycle Fluctuations in Risk

This is an IAPMD Professional Community Webinar and the content is aimed at Health Care Professionals & Researchers. Patients with a special interest are welcome to attend but please note, that the presentation will be clinical in nature and we are not able to answer questions about individual health concerns/circumstances.

This session will present an emerging framework for understanding when individuals may be most likely to experience STB*s and who may be at the highest risk.

We posit that, for some females, STBs are associated with neurobiological sensitivity to hormone changes across the menstrual cycle. Specifically, we hypothesize that exposure to normal hormone flux during the weeks before and during menses (i.e., the perimenstrual weeks) is associated with a rapid co-escalation of negative affect and disruptive social experiences; these hormone-related symptoms may individually or conjointly increase acute STB risk in at-risk individuals.

We outline both an explanation for which individuals may be at especially high risk (i.e., between-subjects factors) and potential mechanisms to explain the association between the perimenstrual phase and females’ increased risk for STBs (i.e., within-subjects factors). Data will be presented from three studies designed to address different tenets of this model, using rigorous methods to advance the extant literature on suicide risk and the menstrual cycle in both adults and adolescents.

*Suicidal thoughts and behaviors.

Speaker: Sarah A. Owens, MS is a current Clinical Psychology Resident at Brown University and will complete her postdoctoral fellowship at Brown University in the Pediatric Anxiety Research Center next year. She will receive her doctorate from the University of North Carolina Chapel Hill, where her NIMH-funded dissertation investigated menstrual cycle exacerbation of suicide risk in adolescents.

Interrogating The Role Of Sex Hormones In The Human Brain - Professional Community Webinar

Sex steroid hormones regulate regions of the mammalian brain that support a multitude of higher-order cognitive functions.

About this event

Sex steroid hormones regulate regions of the mammalian brain that support a multitude of higher-order cognitive functions. In this talk, Dr. Emily Jacobs will share recent findings from her lab, which uses multi-modal brain imaging, endocrine, and computational approaches to understand the extent to which sex hormones influence the structural and functional architecture of the human brain.

Major initiatives include the study of neuroendocrine aging during the midlife transition to menopause, pharmacological studies of gonadal hormone suppression, and dense-sampling studies across the menstrual cycle.

Speaker: Dr. Emily Jacobs is an Associate Professor of Psychological and Brain Sciences. She holds a B.A. in Neuroscience from Smith College and a Ph.D. in Neuroscience from UC Berkeley. Prior to UCSB, she was an Instructor at Harvard Medical School and in the Department of Medicine/Division of Women's Health at Brigham & Women's Hospital.
